if your cursor disappears try hitting TAB to show the scoreboard, that usually fixes it for me...

Well you seem to have tried everything to get it to work to no avail, what you can do is directly report it via Origin and see what can be done from there as well. Open Origin, click on Help then on Origin Error Reporter. In the Type of Error category, select which one applies [believe it fits under the Other category] then describe exactly what the issue is and what you cannot do.
Just by any chance, when it shows you the icon to self revive, does it show you how many you have left?

Yes, I used the repair option. May be a problem with the preload version of the game ?
-- Just gave it another try, starting the 64bit-exe with admin rights directly from the folder: First Self Revive - keyboard worked
Second Self Revive - only by Controller-Button
Third Self Revive - no effect by keyboard or Controller
