My rule of thumb is to never refuse a revive if someone start one, even if it rather pointless or dangerous, because it give them xp or even in dangerous situation,sometime they can still manage to revive you somerow hue. Also If I wait for a revive or not, generally if I see someone in the enemy team walking in my body when not under fire, I know I'm not gonna get revived so I just respawn.
Also in team vanquish or when playing a legendary variant you're almost always better off waiting for a revive. (remove a point for TV, keep meter for legendary variant)
As for when I revive...Well if I can revive someone without exposing myself or if getting said revive is important,might as well. But it case specific most of the time. As a healer,you obviously want to revive as much as possible too.

The bigger question here should be why are people not reviving the healers who can then heal you to full health?
I have seen it time and time again, Being walked all over by a team mates when I play a Sunflower. Which is beyond stupid when they can be your partner in crime and stay by your side and keep your health constantly topped up.

To get back at the player who too me down. I want back at them NOW, not when I might see them again. I'll gladly take that revive.

What I do when I'm vanquished is press the respawn button as fast as I can. I only take revives from sunflowers and scientist if they are close. I hate when the game lags and some body who isn't a sunflower or scientist revives me. Also I never revive unless the quest board gives me a challenge to revive.
