I'd guess you've experienced the Hero Selection BUG on Heroes vs Villains. I made a report back in June about it: https://answers.ea.com/t5/Bug-Reports/Unable-to-Spawn-on-Heroes-Vs-Villains-Hero-Selection-BUG/m-p/6840159#M16091
However it must happen in a way that when you come back to selection screen, there ARE heroes available, but as soon as you pick one, instead of sending you to the game it keeps you stuck in selection screen now with all heroe sgreyed out as if there are no more available heroes to pick. This used to be fixed as soon as a teammate returned to selection screen, but last time this happened to me it fixed itself not long after it happened (and no ally had died until I spawned).
If when you die and go to selection screen, heroes are already greyed out and you can't pick any, then this is a new BUG I'm not aware of
