For those of you who had this issue, has the error gone away?
Same here (Belgium). Before the update I had the error only when I exited the matches but now it happens in the middle of the game...Getting kicked out with zero progress is a mess.
Same story here (Philippines). Before the 450mb update - I was one of the few who haven't experienced any issues whatsoever upon initial launch. After the update - everything changed. I ended up getting conked out while in the middle of a game. Also noticed all the weapons that I managed to unlock through grinding were gone. How often does the bug occur? Every time (100%) - this started to happen after your 433.60mb patch (11/21/2017 - 17:01PH - GMT+8) - see attached image for reference.

In what game mode did the bug occur? Galactic Assault
What is your time zone? GMT + 8
Steps: How can we find the bug ourselves? This happens whenever I try to join any match at any given time, especially on a Galactic Assault game.
What happens when the bug occurs? I get kicked out from the server while in the middle of the game.
What do you expect to see? I shouldn't end up getting kicked out in a middle of the game.
