Yes this is still happening
That's my assumtpion as well, glad you believe it's the actual reason. I haven't played many GA in this game, I'm mostly an HvV player. But I realised by seeing a few footages I have, that this only happened when both Objectoves of the same phase were completed almost together.
I'd guess the routine to remove the marker is overridden by the other marker being completed, and with 2 markers and only one routine to remove a marker, one stays on forever.

I never had the change to see the exact moment this happened, since I always see it too late to record it, @RogueZeroRendar
However it seems a logical reason. Maybe some system that resets all the previous objectives of completed phases should be run every time you go to the next phase.
The bug isn't very common but is pretty distractive since I always look to it (as if it is a new objective-update) and it is full in your screen. Also, you cannot really read any other updates on the current objectives because this is placed through the text.

@DYSPROssium- This big has been around since day 1. It was also appearing occasionally in the prior SWBF. It's common in Yavan 4.

@MasterAmras is correct, I'd like to bump this because I experienced this again while playing with @BS-BloodSpiller last month, and it happened on both our games, and both phase objectives on Hoth were completed simultaneously.
This seems to be a simple programming mistake, like I described before: the routine of a phase objective completion overrides the previous routine for the other phase objective completion, which ends by leaving one of the markers stuck on screen until the match is over.
As these markers are big, solid, and take precedence over any other element of the UI, it disturbs players who are affected. Seems to be an easy fix to me, I hope it's caused by the interaction of the game's code elements, and not something internal to Frostbite (like the Sound BUG).
