Hey jeffbgoofy - I got a response from PS UK on Twitter. They suggested re-starting my PS4 in safe mode to see if that would help with the install. So far so good. Have a look at this;

The error you are seeing is one that pops up on the PS4 from time to time, it can be due to a number of things. If playing on a disc the first thing to do is check the disc for any scratches or smudges, I fixed the issue with one of my games a while back simply by giving the disc a wipe. If the issue remains and you are using a disc, first delete the game data and once done eject and reinsert the disc and it should start up.

Community Manager@DarthMasonLydia If you are seeing the same issue then freeing up some space on your hard drive should fix the issue. The pause icon is just the consoles way of letting you know that you do not have enough free space.

Community Manager@TheKingInTheNarf Have you tried to free up some space, remember that with the PS4 if a game is 50 gigs then you need 100 gigs free so as to download and then install the game.
Community Manager@TheKingInTheNarf Give the disc a clean with a soft cloth, a fingerprint or bit of dust on the disc can cause issues.

I have seen people fix issues like this by re-intializing the console but doing so will wipe.
