Unable to access newly unlocked weapon mods in XBOX version.

Summarize your bug I have been unable to access/unlock any weapon mods that I did not previously have unlocked. For example: I just started playing again after a long while and am unable to unlock the exploding shot for the BLURRG-1120 after meeting the requirements. The Lock symbol is gone and the mod tile is no longer grayed out. Same situation with the A280-CFE Ion Shot mod as well. However, I am also receiving error code 2,495. Not sure if it is related or tied to the challenges....

Steps: How can we find the bug ourselves? Receive any milestone unlocking a mod for any weapon.
What happens when the bug occurs? I cannot select the mod in question or use it.
What do you expect to see? If working, the mod should be selected and usable in game. If not, then it would not be possible to select it.
