Exploration blocked in combat mode

I am having several technical issues on the game with items that I cannot collect, for no reason in the first video there is a force totem that is in the Koboh outpost which is obtainable after defeating an enemy out I beat it a long time ago and since every time I come back even after reloading the area and even restarting the game, the totem tells me that it cannot be recovered because I will be "still in combat" which is wrong in the second video locate Jeddah a door.
