Locked in combat, can't collect/scan/etc.

Summarize your bug I currently get stuck in combat in the basement of the Koboh house where the man killed his brother, and just above that location, outside the house that is covered in Koboh Webbing that needs to be destroyed with the laser and Koboh Grinder. This makes it impossible to collect the reward for entering the house, and incredibly difficult to solve the puzzle with the Koboh Grinder as Cal cannot use the grinder while in combat. I have tried reloading the area and returning later in the game. This bug was present before I ever entered the house and triggered the initial fight with the man inside. I killed the man with a blaster bolt while he was running back around the corner, so that may have something to do with why the game thinks combat needs to trigger there, but he is not respawning.
How often does the bug occur? Every time (100%)
Steps: How can we find the bug ourselves? I'll upload my save so that it can be looked at in the compose field below, as I don't know if it's easily replicable on other files. My best guess to recreate it would be to kill the man in the mystery house with the blaster stance while he is down the hallway.
What happens when the bug occurs? Cal is stuck in combat in a certain part of Koboh, as shown on the attached images below
What do you expect to see? Cal is not in combat and able to solve the puzzle with Koboh Matter, along with collect the reward from the house.

As you can tell from the HUD in the images, Cal is stuck in combat despite there being no enemies nearby, the man who attacks you in the basement is gone, and the rewards are inaccessible. Save file attached as well to help potentially hunt down the root cause of this bug.

Steps: How can we find the bug ourselves? I have attached an image of the error.
What happens when the bug occurs? Can't collect the upgrade.
What do you expect to see? The upgrade available for collecting.

• I have tried going to a different location or even world.
• I have logged out of the game and the EA app to fix it.
• I have tried repairing the game in the EA app.
• I have tried shutting off my computer and coming back in. 
• I can collect other upgrades around the maps but not this one.

Steps: How can we find the bug ourselves? in the town someone who kill his brother. that cave fight end. but i can't loot item. said fight not end. end sometime other place happen too.

I have the same problem. no matter the key binds on the controller or keyboard I use, I still can't use focus. I am also currently stuck in a chamber where I cannot leave without using the focus key button. I hope there's a patch that comes out soon to let me leave so that I can continue the game.

Also occurring on Xbox, so doesn’t Sound like platform specific bug.

Summarize your bug Stuck in Combat (Formerly "Focus Option Sometimes Disappears")
How often does the bug occur? Often (50% - 99%)
Steps: How can we find the bug ourselves? I guess play the game. Not entirely sure how to reproduce it.
What happens when the bug occurs? Periodically while playing the game, I get stuck in combat. This makes it so that the focus option is unable to be used, i.e. force lift, force slam, confuse. This happened to me twice, once during a section of the game preventing progression (the second moon base run through where you need to lift the wall) was the first time. 'Resting' at a Meditation Point did not fix it, nor did going back to the loading screen and re-loading where I was. This second time it's occurred is just out in the wild while trying to lift and slam a perk container. Closing and opening the game fixed it. I DID change my default key binding for focus to tab, but have not had a problem up until the patch today.
What do you expect to see? I expect the game doesn't lose the ability to use the 'focus' keys.

I included screenshots because originally I thought the UI element on the bottom right was also not showing up, but it appears that element typically goes away once out of combat. In the first screenshot it was when hitting focus was not working, second is when it is working.

Edit: It appears instead that maybe I'm stuck in combat? It's not just focus. It also seems to be effecting BD-1's BD-Noculars. I assume there is supposed to be something done with him at the third photo which is in Imperial Post 8L-055.

Cal leaves the saber out despite combat being over. I can manually put it away, and the interface gets "clean" again. In the third photo I was able to jump off the balcony enough times to get a 'respawn' screen, and afterwards was able to use BD-1.

Summarize your bug Unable to pick up an essence in Rambler's Reach due to a combat state even after the enemy was defeated. It says, "unavailable in combat' even after reloading the save and rebooting the game. Another is an unseen scan that presumably due to the same bug prevents me from scanning it in Hunter's Quarry. The last achievement might also be bugged for me since it should automatically unlock after reaching 52/53 achievements. Neither the EA app nor Steam unlocked it.
How often does the bug occur? Every time (100%)
Steps: How can we find the bug ourselves? Unsure. The first one happened again even after a reload.
What happens when the bug occurs? Unable to pick up/scan these 2 objects.
What do you expect to see? To be scannable and picked up outside of combat.

Summarize your bug In the side mission "Death in the family", the player may be unable to pick up the upgrade in the basement if they listened to the echo first. The item will always display "unavailable in combat", even after restarting the game or travelling off planet.
How often does the bug occur? Often (50% - 99%)
Steps: How can we find the bug ourselves? Open the bunker in Rambler's Reach, Go to the basement, Listen to the echo, Kill the brother once he attacks you, Try to pick up the upgrade in the room.
What happens when the bug occurs? A message "unavailable in combat" is displayed when trying to pick up the upgrade.
What do you expect to see? Being able to pick up the upgrade once the combat is finished.

During the side mission "Death in the family", I opened the fortified building in Rambler's Reach. I calmed the NPC upstairs and then went to the basement.

There were an echo and an upgrade. I listened to the echo and the character from upstairs came down and attacked me. I killed him and then, I was unable to pick up the upgrade once the combat was finished. I went off planet, restarted the game, but that upgrade is stuck in combat mode.

As far as I can tell, it has not affected other items (yet).
