Confirm button frequently doesn’t respond (have to press X multiple times)

I’m experiencing a frequent input issue on PS5 where pressing X to confirm an action doesn’t always execute the command. Sometimes it works normally, but other times I have to press X several times before the game finally responds.

I’ve also seen other players reporting the same problem on Reddit, including Xbox players.

Platform: PS5
Issue: Confirm/action inputs intermittently fail to execute
Frequency: Very frequent

Has anyone else on PS5 been experiencing this?

    I've seen this on PC with MKB / Space as well.

    I've also seen the camera control get stuck on steam PC controller buttons on steam link.

    agreed, space bar to confirm simply doesn't work sometimes (seems inconsitent). Clicking with mouse seems to work more consistently, although sometimes it still takes a second click.

    To confirm, I have this issue on PC as well. It happens whether I'm using an XBOX or PS5 controller or a keyboard. It happened a couple times in the weapon mod menu the first time I had a mod, so the tutorial message was there. No buttons would respond except for Y, so I was stuck in the menu and had to force quit.
