I at least figured out why this is happening. I've been monitoring the actual save file behavior and it looks like the file will delete before it actually has a new save to overwrite it. For example, when finishing a mission, the save deletes before the transition cut scene and then doesn't create a new one until you're back on the ship, so if the game crashes between those two points after a mission any iron man saves are destroyed. Given the common crashes, this is probably a pretty high priority to address if you want iron man to be a part of your game (which I believe it should be).
