Depends on the game, I think. Mine tend to be homeless NPCs more than any other, they keep getting pushed to age up and staff their assigned role objects at the same time. But inactive sims can't age-up on screen so they end up stuck and sometimes unable to do either.
For resident sims, either bad timing or yes the outfit assignment (could even be a "bad" career outfit) or bad CC.
