In the past I've turned them real with the potion from the chemistry set and kicked them out like any other sim. That's at the YA stage. Otherwise I'll have the child/teen sim ask them to go away and they'll turn into a doll which I'll stick into their inventory.
I didn't like the IMs overall. They are nice when the sim is a child - he or she always has someone to play with. But as teens or older there should have been a "goodbye" option that would send the doll away. I eventually got a mod that stopped babies from getting them in the first place. I tend to play large overstuffed families and having so many IMs would create too much chaos, particularly in regards to routing.
