Seems like we're talking mostly everyday wear here. For each life stage (starting with child anyway), I usually make a warm weather and a cold weather outfit for each sim, plus a third that's "different" from their usual style. If they're pretty casual (most of mine are), I might make something a little nicer to go out on the town (short of formal occasions), or if I see them as looking, say, more professional on a day-to-day basis, the third outfit would be their dress-down choice.
For other clothing categories, I almost never make more than one choice of outfit per life stage. I do change most outfits on a birthday, although if I think one they currently have is still OK for where they're at in life and I'm not terribly tired of it, I'll leave it alone (I guess that's only possible/likely between YA and adult).
