For example, in my current game, the granddad is still making the potions for the extended family, even for other households. One of his grandchildren just discovered the potion himself (as his second potion, no less), so I should be set for a while.

The vanilla way to achieve this is to just gift the doll to another sim. That works. Do it before the game generates the sim that accompanies the doll.
Once it's in their inventory it's considered theirs, so how would you gift it to another Sim before it gets to their inventory if that's what you mean?
I did the same thing with one of my households. I had no problems moving the potion from the making sim's inventory to the other sim's inventory. I just dragged it across.
It was in another game where I had a lot of Imaginary Friends and the game had been running for a long time and they had millions of simoleans and I'd already had a lot of sims make their own potion and was tired of it and one sim had found all of the potions and just seemed to love making potions so I got her to make a few extra imaginary friend potions. They cost over 4,000 simoleans each but that cost didn't make a dint in their wealth.
Oh, yeah that's true you could move the potions to another Sims inventory. That seems like a good idea when you have a lot of Imaginary Friends in the game and end up with a lot of the potions. Of course it would have to be a Sim that was already in the household to move it to another Sims inventory but if you wanted it to be for a Sim not in the same household maybe you could gift the potion to another Sim that's from another household.
Or you could temporarily move that other sim to your sims household.
