Sorry you didn't get a response before now. I'm not sure about the first question—I've never actually played with a fairy—so I can't help you there. But if you use mods, you could try resetting the fairy with NRaas MasterController, and if that doesn't help, use MC to knock their skill down to 9 (or lower) and then raise it to 10 again.
For the second one, some worlds just have fewer spawners than others. If it's really frustrating you, just put down a couple of epic beetle spawners on one of the empty lots and see what happens. You could also try searching the world lot, as in, the space that isn't part of any lot: click on the ground, select NRaas > MC > Object Info, then look for epic spawners.
For the third, I've often noticed inventory failing to refresh for a few days, but it always corrects itself. If it continues to be a problem, just bulldoze and replace the lot, with a save/quit/reload in between.
