When I was attempting TS3 tried all the sliders and what not available (tis a fine art in TS2), and they still look the same. Any differences I got were merely superficial. One of the root problems is that the human head is not a ball, but an ovoid. No way you can build a good house if the foundation is flawed. Suspect that to get any meaningful variations would require mods/CC.
Wrong, sims heads are not one shape in 3,sims 3 head sliders include
For cheeks
Cheek fullness
Cheekbone height
Cheekbone Shape
Nasiolabial crease
Cheek jowls
Jaw
Jaw width
Jaw shape
Jaw Height
Jaw depth
underbite
Chin
Chin depth
Chin height
Chin scale
Chin underlip scale
Then with that globally
Face Profile
Head width
Face height
And 6 ear sliders
Also there are 12 different preset head shapes to start with
I do not use CC or sliders/mods this is a pic i took half hour ago of a sim I made the other day
