when you unlock this item, it starts appearing in your default stage setup so that's normal it can also be accessed from the inventory, but it's a hassle to put them down for a couple of tricks check your personal/family inventory if it's in the family one place it on the ground and  have the Sim pick it up

or try but do save your game first because if it doesn't work just quit

to the best of my knowledge no cannot be purchased and  unfortunately, the  item is not in buydebug, check the family inventory in case it went there 

 sometimes you can get these items back by moving the Sim out via edit town and moving them back in or by traveling to a vacation destination if you've got world adventures

the other thing you can try is use a cheat to slide the career  skill down to one  Ctrl + shift + C type in testingcheatsenabled true

you then click and drag the job experience bar your sim has and basically demote them  then slide the bar up to promote them so hopefully they get the balance ball back  at the career level it was most important turn the cheat off after that Ctrl + shift + C type in testingcheatsenabled false
