We've been unable to reproduce this issue internally, if you are still encountering this on the latest game version (without mods), could you please provide screenshots or save files (save file instructions here http://x.ea.com/74420) of it occurring in-game to assist with our investigation?

@EA_Cade I think this might be to do with whether you clicked "apply all" from the everyday outfit or from some other outfit. For example, if you happened to age up your sim in a winter outfit, then change the hair on that outfit using the 'apply all' then it doesn't change the bathing outfit. But if you do it from the everyday outfit using 'apply all' then it DOES change the bathing outfit. So that would explain why some people see it and some don't. It depends on whether you changed it from the everyday outfit or something else (in my case I aged up my sim using his winter outfit when it happened just now, but then it fixed from everyday, but I could reproduce the issue using 'apply all' from the winter outfit).
