Annoying bug that took me a while to locate. If a fireplace is upgraded with auto light, and the fire goes out. That fireplace will give a filthy surroundings debuff to neat sims who are in the room with the fireplace until the fireplace is deleted. This does not seem to bother non-neat sims in the slightest. At least the fix is simple, don't upgrade your fireplaces with auto light if you have sims with the neat trait.
This is still an issue.
I replicated it following these steps:
1.- Make a sim upgrade a fireplace (I used the new GT fireplace) with auto light
2.- Once upgraded, I sat a couple of sims in the room, one of them neat. They had a +1 positive moodlet because of the fire.
3.- I turned off autolight. A negative moodlet appeared telling me the room was dirty, only for the neat sim. Turning on the auto light again didn't delete the moodlet. So now each time my neat sim is in that room, she has a negative moodlet.
4.- Going out of the room makes the moodlet disappear.
5.- As fakeplasticadam reported, deleting the fireplace seems to be the only way of getting rid of that moodlet.

I don't have a save game with the issue happening sorry, I purged all my saves of neat sims a long time ago. Maybe someone else can help?
