On case you can't tell, the bladder meter there is full, but it stays red and the moodlet doesn't go away. Restarting the game seems to be the only way to fix this, because using cheats to max the need out just nets you the above screenshot.
Tested this, have no issue.
Did your Sim go to the bathroom or did they pee in their pants when it didn't go away or was it the cheat that didn't make it go away ?
