I have the same problem. My start sim first had a baby with the girlfriend. The baby was born when they were living together, but they were "only" a couple, not married. However, both are the parents of the child in the family tree. Specifically required baby care tasks were even checked off as done before the birth, when my protagonist & his pregnant girlfriend visited friends with babies and interacted with them. The 'having baby' part is given as 0/1, even now after they have had twin boys. All other tasks are done except for the teenage tasks. The first-born child has currently just aged into a teenager and the tasks can be done. So only the "have baby or adopt" part remains open.
