It seems to happen with all sorts of things that the sims are currently holding. My teenage sim did (one-handed) push-ups while she was still holding a cup of coffee in the other hand (I don't think she had actually finished it, so the bug is that the game didn't force her to put it down first). I happened to save right there, and the next time I started the game the cup was gone and she continued her push-ups two-handed.
