this is an old issue that seems to have resurfaced for some with the newest patch.
If you're not using any mods, try making the kids leave early. That usually solves the issue.
It DOES seem that this issue is back (although I never experienced it the first time around). I thought it was perhaps an issue with one specific household, but it's not. It seems to happen mainly to my teens coming home from either work or school. They physically arrive home, but their icon is still grayed out as if they're still at school/work. And they don't seem to follow direction in this state (I know what they say about being physically present but being mentally elsewhere, but that's not so fun in the Sims!).
It is possible to have them leave work or school early to "fix" the problem (although it doesn't work for me until after I've either reloaded the game, or re-entered the lot from manage world), but not without consequences. If you wait too long to do it, their needs drop drastically. AND you also wind up with a performance loss as the game thinks the sim is quitting early when really they've put in overtime. My one teen is now an F student because of this repeated problem, and he's not doing so hot in his job, either!
Hope this gets fixed in a patch ASAP!!!

Will do! I had those teens quit their jobs and it hasn't happened since, but I'll reemploy them and see if I can get it to happen again. If I do, I'll post it for you and share the link here!

Hey, I'm good to go to share the DxDiag and a save where this is happening, but you'll have to refresh my memory on how to do it. FYI, this time around, my teen (who has school and a job) got stuck like this when she decided to play hooky. Instead of going to the lot with her friend to play hooky, she instead has been sent home, but her icon looks like she's still at school. I see you've attached instructions on how/where to upload a file, unfortunately, I don't know how to get/create/find that file to upload! Let me know and I'll get it to you asap!
FYI, I'm on a Macbook Pro, so I'll also need a little help figuring out how to create/find a DxDiag file on a mac. I haven't created one of those (or uploaded an bugged saved file) for eons, since I was playing Sims 1 on a PC!

Hi @ceb6973, your save files can be found by going into Documents -> Electronic Arts -> The Sims 4 -> saves. If you sort the folder by Date Modified, then the most recently modified *.save file will be the last save you played.
For your DxDiag, that's actually called System Information on Apple.You can go to Apple menu -> About This Mac --> System Report and then choose File -> Save to save a copy. Here's more info: https://support.apple.com/en-us/HT203001
