Bug: Sim Body Morphing After Traveling

How often does the bug occur? Every time (100%)
What is your current game version number? 1.110.311.1220
What expansions, game packs, and stuff packs do you have installed? All packs excluding: Lovestruck, Cozy Bistro, Riviera Retreat, Artist Studio, Storybook Nursery
Steps: How can we find the bug ourselves? Enter CAS. Change sim's face and/or body. Exit CAS. Use phone to travel to any lot in any world.
What happens when the bug occurs? After leaving CAS and traveling, the Sims' body will morph/stretch. The face will also scrunch or stretch.
What do you expect to see? The Sims' body to stay the same after leaving CAS and not changing from traveling to a new lot.
Have you installed any customization with the game, e.g. Custom Content or Mods? Not now. I've removed them.
Did this issue appear after a specific patch or change you made to your system? Neutral/Not Sure

Image One: Teen Sim is outside of house. Body and face is stretched and scrunched.

Image Two: Same teen sim is in CAS with "fixed face" (nothing altered from the face preset but the eye color) and "fixed body shape" (shoulders widened, hips widened, for example)

Image Three: Same teen sim is at Isle of Volpe Park and the face and body reverted back to the pre-CAS body and face.

My teen sim is a (science) child of the Grim Reaper. The Save was made prior to the new expansion pack 'Life and Death' but she was born after the game was released. Her face was always somewhat scrunched, but I suspected it was because the current save had the "old" Grim face and that was just his genetics but after aging her up to a teen and changing her appearance in CAS (on multiple occasions and saving) her body and face keep reverting back. 

I thought it was because of the new eyes from the Grim Rewards but changing to the "normal" eyes still makes the face and body change. 

I also thought it was because of a mod or some CC, but I disabled them and restarted the game twice and the bug still happens.

I have repaired the game as well and it still happens.

I am unsure if it is just the save or if it happening on the PC as well. I will have to check and see.
