I have this happening in my current save. I have two evil chickens and the grim is stuck in a loop. I was able to stop it once by adding grim to family and then moving him out (but then if anyone else dies again it will just be stuck again). Once I locked the door to the chicken coop and grim left, but then it came back (at this point I'm not sure if someone is dying or he is just stuck). Even if you somehow manage to fix it, every new death just results in him getting stuck. Also, sometimes I am able to save the file, but sometimes it says that I cannot save the file while sim is dying.
