Steps: How can we find the bug ourselves? Have an alien sim in the doctor career and join them at work.
What happens when the bug occurs? The sim does not change into work outfit, while human or alien.
What do you expect to see? I expect the sim to wear the proper work outfit.
Have you installed any customization with the game, e.g. Custom Content or Mods? Never used.

The sim is created as an alien, they started the doctor career path and has not worn the appropriate outfit at all. They are currently an Assistant Nurse in the career. The only time I see the work outfit is when they get a promotion. The outfit they wear to work is a bra and panty with the work outfit shoes.
