This has happened now with a couple of my sims.
When they get pregnant they don't have a belly for the first trimester, they get the pregnant icon and everything, just no belly in the beginning.
I am NOT using mods or custom content.
I have also repaired the game.
I'm unsure how it's supposed to work. I always get this too and I never considered it a bug. Others have reported this as bug since they don't seem to get it anymore (thread around somewhere).
What posture does your Sim have ? Do you get the same with all Sims also if they're really skinny ?
Depending on their fitness level, natural stomach shape (super flat stomach) matters on how they're bump will look. I have noticed too that when my Sims are carrying multiples their stomachs are bigger than usual.

Crin, I've had both issues since the launch of the game on different female sims.
The first question for the developers is:
Should the baby bump appear in the first trimester?
If yes, then sims not getting the bump is the bug.
If no, then sims getting the bump is the bug.
Personally, I don't think they should show a bump in the first trimester. I think the majority of women (unless it's a multiple birth pregnancy) don't start showing until at least three months.
If the developers have randomized the bump showing or not showing in the first trimester, we just need to know so we don't think it's a bug either way.
