I have same problem with two dogs. Both of them are Smart. One is Smart, Loyal, Active. The other is Smart, Loyal, Sleuth. Both were created with the sim in CAS originally as puppies. Both of them have a friend dog who isn't Smart, so there's comparison pupster in the same household.
The Smart ones will not learn to pee/poo outside on their own. They will complain and whine at the door until taken outside and asked to go to potty.
The non-smart ones learned already as puppies and have been going out to do their business ever since.
The Smart ones learn lectures in one go (non-smart ones take two or three times), but they just won't learn to pee on their own.
They run outside otherwise on their own as they please, aka they go out just fine and play and run, but when it's time to pee, they come back in and whine at the door.
