So I get this issue but what happens for me is that my male child sim has the loose tooth soon buff, and randomly will change pitch black in skin color with random clothes on but those clothes are the same in each outfit category. For instance, sweatshirt and sweat pants in everyday, then same outfit will show up on sleepwear, swimwear, etc in every category.
My male child sim is not aging up yet, he's still in the middle of childhood on normal, did not lose the tooth yet, and did nothing to get rid of said tooth. He just got the moodlet of hey, loose tooth kiddo! And that was it.
This has happened in my current save with mods, ALONG with a PREVIOUS save with NO MODS ever to a different female child sim that I was playing. That previous save is now gone (been deleted) so I cannot share it but I had no mods in game, or CC for that matter, and I still got the same exact error.
It was fixed the first time in my non modded game by closing out the game and going back into it without saving. I just got the error with the male child sim in my modded save, and closed out the game, unsure if it auto-fixed itself again or not.
So it is not a mod issue, since it happened in a non-modded save before, it's with the EP itself probably since it's a feature from it. Have not had this issue with teens yet.
For reference, I am on patch 1.96 on PC with all packs and kits.
EDIT: So my male child sim will NOT change back, and I assume my female child sim did not either, she just reset herself at the time and when I went in game I failed to notice her face changed. Oops. So yeah, no way to fix this other than modding it of course. Or EA fixes it. So I just went back a save, fixed it with a mod I made temporarily, and it didn't happen again. Yay for temporary fixes for now lol.
