This should be fixed with Patch 1.5.149

On the second day of work at the hospital the Doctor stayed as you said. However, when I went back and played the Doctor left early on both days 3 and 4 with no other Doctor to replace her. My sim cannot transfer cases and therefore cannot continue in the Doctor career. He will have to quit and find something else. This has happened with several sims in different worlds and after trying several different solutions (I also have no CC in my game) and also seeing that this problem is not unique, I can only think that it is a game glitch. Hopefully we can get a resolution soon because this is my favorite career path. Note: this was not a problem when I first downloaded Get To Work. I cannot remember whether it was after an update or a stuff pack but I know it has not been a problem since the start.

You don't necessarily need to do the task to move up. Try to do all others, IIRC they should change each day.
You can also consider a cheat and leveling up: Careers.promote <NameOfCareer>
