Steps: How can we find the bug ourselves?
i did this in a new save, new household but this is probably not required.
in cas, make a spellcaster and proceed to add new sim via genetics. randomize sibling or child. so long as they are teen or older, you'll probably eventually get one of the unremovable skin details. click the face button that removes all skin details [you can also do the same for the body if you wish]
What happens when the bug occurs?
the sim has what appears to be at least 1 skin detail regardless of me clicking to "remove all skin details"

all skin details removed when i click the button to remove all skin details
Have you installed any customization with the game, e.g. Custom Content or Mods?
no. it's been years since i've used cc or mods.
Did this issue appear after a specific patch or change you made to your system?
no clue, this is the first time i've gone to play sims 4 in years.
hopefully i've described this adequately. i am unsure whether this should be grouped in with the mortimer skin detail thread or not so i made a separate one as i haven't even looked at mortimer in this save.
edit: possible additional relevant information, clicking a bunch of skin details and/or makeup does not replace or make these skin details(?) go away. it also seems like it might be inheritable OR it's a matter of the frequency it randomly applies to sims.
