This seems to be fixed for me with the latest patch (although it wasn't in the notes). Maybe someone else can test/confirm it?

happened to my sims aswell, not sure if it's connected to the happy moodlets, i'm seeing my sim as disfigured in the relationship panel too
I have the same issue, only my Sim now looks old and she's blonde! Initially it transformed her actual body too but after a reload it's just her avatar. I don't use mods and this is the first time it's happened (multiple tans before this in the same gameplay).
