If I import a household from the Gallery directly into a lot and save the game, every Sim in that household's muscle and weight resets to 50% on both sliders. This occurs immediately after saving. Note that if I create the household in CAS first, and move that family into a lot when prompted, their muscle and weight is unaffected by saving.
Also note that if I save the game from Manage Worlds before entering the lot, the muscle/weight does not reset, but will reset when saving following my entry to the lot. Furthermore, if I enter the lot and return to Manage Worlds without saving, the muscle/weight still resets.
This appears to be a one-time issue for newly-imported households, as any body changes that occur afterwards are maintained when saving.
This issue occurs without mods.
I've that problem too. Except, my Sims are from my library. But everything else is the same as written from LegionLG.
I got around the bug with loading my Sims into the CAS as "new household" and place them somewhere in the world afterwards. Or with another household I've used the "cas.fulleditmode" after they got their bodies reset. Then they stayed the way I've made them.
It really seems to be a one-time problem. Hope it will stay this way and doesn't get bigger, also EA/Maxis will hopefully look into that one.

This is still happening when I place my Sims from the gallery. I've tested it some minutes ago (also without any mod stuff in the game).
Hope it will be fixed since the work around is still putting the Sims as a "new household" into CAS and then put them in the game.

Do they only loose them in live mode (in which case this would probably be the same: http://answers.ea.com/t5/The-Sims-4-Bug-Reports/WORKAROUND-Muscle-tone-not-properly-displaying-in-Live-mode/td-p/3485621/jump-to/first-unread-message or also in CAS ?

To me it's in the live mode. I've tested it with placing a couple from the gallery which is thicker so you can see it really good that they just get a reset to slim Sims when I save the game. (Both muscles and body weight are in the middle of the sliders.) Actually I don't know what happens when you have a Sim which is already quite thin but guess it'll be the same.
I've made screenshots of the sliders so I could put them back to their normal size which I had for the Sims in the first place or, like I said, put them in as new household. But guess it wasn't intended to do it that way all the time (when you need to de- and re-install the game or place Sims from the gallery right into the game).
Don't know if it's related to that in the link you've posted. Maybe it is or it's at least similar.

This still happens with sims placed in the gallery even after last patch. now i have a theory and i am wanting to test it. I make sims. There are times when i make two versons of a sim. The first version does not seem to be affected, whereas the second version, especially if it has been re-redited in CAS is.

This same issue is happening to the sims i have shared on my gallery. I mean its a matter of going to the gym a few times to gain the muscle back but its a pain.

I have this too, for both the weight and the muscle. It's been that way in my game for quite awhile, it's not a new thing for me.
When I use the gallery/library, and place a sim into a house, then in live mode their sliders have reset to the middle. I go into cas.fulleditmode and change them.
Recently, I had a "simself" party, and downloaded a large amount of people's character. THey all went to 50/50! I had to start over, take a picture of their sliders, then go into their house, bring them back into CAS and adjust it back.It does work fine as a workaround, but would be nice if I could avoid the bug in the first place.
