I have the same issue, {currently researching while Focused}

Which language are you playing the game in? English or a variation there of {Simlish}
How often does the bug occur? current world has mishapinings in Family Tree , new world as of yet has not experianced the same issue {knocking on perverbial wood for luck}
What is your current game version number? orgin keeps up to date in auto-update mode,
What expansions, game packs, and stuff packs do you have installed? everthing except that new fashion thing {Sorry Milan I have failed thee}
Steps: How can we find the bug ourselves? have a few families have kids and those kids have kids,move out family that is just taking up space into the world elsewhere,
What happens when the bug occurs? Sims are removed, with a blank picture in place of sim, like say- the mother of my Sim's son, or the mother and grandmother of another household that I hadn't played in awhile,
What do you expect to see? Sims not removed from the game, but a simple picture remaining after they are removed from the game would be sufficient,
Have you installed any customization with the game, e.g. Custom Content or Mods? Never, I don't believe in that stuff... I'm an original Sim player, Sims, Sims 2, Sims 3, and Sims 4, including but no limited to other EA releases such as Medieval.
Did this issue appear after a specific patch or change you made to your system? Neutral/ Not sure, Just noticed that the mother of my Sim's son was gone, deleted, removed, culled, or simply vanished by way of unknown AI ascension to a higher level of existence....
I have spent many hours trying to figure out how to bring the family back together, including trying to save them and bring them to another world, unfortunately even when I bring them in from the Gallery in My Library, the son still has a blank picture for a mother and I tried using CAS to remake the mother but did not allow me to do so.
I have posted to EA Help, but they simple pushed "Closed" and never responded to me, {Excuse the sad moment as I wipe a tear from my eye}
I have researched Sims Forums in vain trying to figure out a fix that does not include MOD - {Modifications Of Death}
Currently creating a new world and {with much Sadness - currently painting a picture during Very Sad moodlet}
In hopes that the new world will be able to have children without missing parents and grandparents or just pictures of those that have gone on to a higher lever of existence.
