I am experiencing this also.
My Legacy leader, who has woohooed approximately 150 times, with at least 20 different sims and had never had an unsatisfactory Woohoo as an initiator is now getting unsatisfied each time she woohoos. I had her, and her spouse, attempt to Woohoo several times and each time she had an unsatisfactory moodlet. She was the primary initiator, but even when he initiated he kept his awesome woohoo moodlet, and she didn't gain one. This is without CC, and in the top quality bed. Her spouse didn't get the unsatisfactory moodlet at all (even though he's on the verge of death). Previously, the one time I'd ever seen an unsatisfactory moodlet was with an elder sim who was the complete opposite of the sim they were woohooing.
I also had my second gen. Young adult sims Woohoo with their new partners, and again, each time, every one (initiators, again) had a negative, unsatisfied moodlet. On the other hand, teens who "mess around" never seem to get the unsatisfied moodlet.
All sims were flirty, all had a romantic and friendship relationship value of between 50-100 (includes everything from soulmates to sweethearts/lovebirds). One couple was married, one was engaged, the other boyfriend/girlfriend. All had performed a series of other romantic actions prior to initiating woohoo. All had at least 1 shared trait, more often 2-3 (it was originally said prior to this patch that woohoo outcomes were related to traits). All were in comfortable beds, and all had high/full needs.
I appreciate the addition of some risk to the WooHoo mechanic, but this seems to have been overtuned to the extent that it's no longer adding meaningful gameplay risk.
Whether Mods are being used or not doesn't seem to make a difference, as it can be replicated with the mods removed as well.
