@crinrict I took the standard hospital from the new save game and put it in my save game. It took a while for the deliver a baby option to pop up, but it did actually work and he delivered the baby. I did however notice with all the testing that they always used the same surgery table for any surgery or the delivery. In the altered version I had that surgery table was moved. I do think that was the problem though. Thank you very much for the assistance my year long level 8 doctor actually got promoted.
@crinrict Oh, well I could try changing other parts of the hospital and only leaving that surgery room standard , in theory that should work as well since the remainder of the career works perfectly.

You should be able to change the location of surgery tables and still have the doctor career/hospital visits work just fine - so long as you make sure you have space around the table so everyone can use those things properly.
I use my own hospital layout and it works great. And I shifted around and added lots of things.
@khitteh are you playing on console? I did move the surgery table, any surgery worked except the deliver a baby hence me not thinking the layout was a problem. Only once I put down the standard hospital did it actually work.
