@crinrict Oh, well I could try changing other parts of the hospital and only leaving that surgery room standard , in theory that should work as well since the remainder of the career works perfectly.

You should be able to change the location of surgery tables and still have the doctor career/hospital visits work just fine - so long as you make sure you have space around the table so everyone can use those things properly.
I use my own hospital layout and it works great. And I shifted around and added lots of things.
@khitteh are you playing on console? I did move the surgery table, any surgery worked except the deliver a baby hence me not thinking the layout was a problem. Only once I put down the standard hospital did it actually work.

No I'm on PC, I also use move objects cheat and do a lot of building - so long as you leave the required space around the surgery table (it should show area on floor that's needed when you pick the table up) it will work fine, same as every other object that needs space around it for a sim to access the thing properly.
Is it possible you have something like ceiling too low for something like one of your low hanging lamps and that was blocking access? What I'm saying is you don't just have to be aware of things on the floor - items on ceilings can also block objects.
