Would be interesting if this also happens to other folks or if it's maybe a problem off cheating the trait.
Can you reproduce this with a regular non-cheated sim too ?

Exactly how would you go about seeing if this trait distorts a body if you can't add it and take it away at will? My copy of Nyon didn't have Grimborn and so I gave it to him with CheatUI and every time he is loaded (as in, save/load, trravel, enter/exit CAS), he gets worse. His limbs and hips are so thin that grabbing them in a cas.fulleditmode tug flashes them red and makes them go to a more realistic size, suggesting that they're being pushed beyond the slider limit. Of course, he gets scrunched back up again after leaving CAS, so it's kinda like a love-letter from the game going, "Thanks for trying, but he's going to permenantly be a disgusting mutant."
Before you ask, no, I will not play without mods, unless you're willing to post footage of undergoing elective dental surgery with no anasthetic because that's what you're asking us to do every time you suggest. You have enough evidence in the rest of this post that this is some straight-up developer-end garbage that's causing slider value tweaks on load. Modders wouldn't selectively single out one sim to do this as a side-effect, it's too much work. This is an EA mistake.
