@Bethfcurry did you have to edit them after removing the trait or did their appearance automatically change?
I had to edit them back but they then didn’t revert back. I imagine if you remove the trait before they age up into a child, that it won’t distort their faces since it seems to happen when they age up. But im not sure. It also still says their father is grim but they don’t have the trait or birthmark anymore after removing it with UI. This is one of my sims that kept reverting back after I entered live mode

Yeah I had my other sim and I gave them the same trait and it messed their face up instantly in live mode it was so strange. Their father wasn’t the grim reaper or anything it was definitely the trait.
