@crinrict Yes, I listened to the plea and then was unable to Reap the sim. More info: when I was unable to Reap I stayed on the lot through until the end of my shift and it ended normally (got paid and tp'ed home). I carried on with the career and had no problems reaping sims on subsequent assignments. BUT, when I received an assignment at the same lot (the Bun Ma household on Hothotok Shore in Tomarang), the same thing happened. This time I tried to Revive, instead of Reap, but the Dead sim was again unclickable. They were standing on the sidewalk in front of the house so I think it might be related to that lot. Attaching a pic so you can see where the Dead sim appears.

Have you done beloved sim reaps at other households since and did those ones work?

This just happened to me as well, but on an empty lot in Newcrest. Had to do an Emotional Reaping. Not the first one, never had problems until now.
Got to the empty lot, the witnesses, the one with the plea and the dead sim were all standing in a group in front of me. Noticed the dead Sim when I hovered with the mouse over her and it said Dead in red. Tried clicking on her, but nothing happened. Continued with the tasks and listened to the plea. Now I can't collect the soul.
I'm playing on a Macbook. No mods or CC.
@TUGASTYLE1997 Maybe empty lots are also an issue as the Sims don't really go anywhere ?
I think personally I only had the fissure/soul one at empty lots so no experience with those.

Would be good if everyone that has this issue would state on which lot they have issues.
I had an empty lot and everyone was standing outside the lot. Nothing around the lot to stop me from reaping the sim or reviving the sim.
update: emotional reap at a different location worked fine
the bun ma household has clearly made some kind of pact with grim 😒
Emotional Reap at a different household worked for me, as well.
