[NEEDS SAVES] [CL] Sim dying from bad pufferfish Nigiri despite not eating any

No they're not and they never have been.

This just happened to me.

My Sim died at a restaurant that serves pufferfish nigiri when I wasn’t playing him, so I can’t vouch for what he was eating. I was playing another household and he was an NPC diner with his wife.

One of my other Sims brought him back by selling him ambrosia through the food stall (and completed the aspiration). I put him back in his household. Everything was fine until I told his wife to eat leftover tofu stir fry after playing them for a week without incident.

She died of bad pufferfish nigiri even though she was absolutely eating tofu stir fry. (Also she was absolutely not pregnant.)

Grrrr. I immediately brought my witch around to necrocall and dedeathify her and gift the family a death flower because this was BOGUS. Grrrr.

(Also my witch turned Grim into a bunny statue. Very satisfying.)

Did something happen?

I’m waiting to roll around to the household that owns restaurant #2, but… I thought restaurant #2 didn’t even serve pufferfish nigiri and I still had an NPC die from bad pufferfish nigiri when I visited with yet another household. I know the chef; he’s a sim I reluctantly marked unplayed because my save is exploding and he’s level 10 cooking and gourmet cooking and has the Fresh Chef reward trait, so even if the restaurant serves pufferfish, how is his “bad”? 

(True confessions: I used a mod to hire the chef.)

Did y’all make pufferfish 100% lethal 100% of the time for Home Chef Hustle or something?

Confirmed: The second restaurant, owned by another of my Sims, does NOT serve pufferfish nigiri. Either the NPC brought in his own food to my four star restaurant (rude!) or the game is just randomly killing NPCs at restaurants because it assumes I find it funny. 

I did edit her menu while I was there to include plant matter ice cream as a possible dessert, as that’s kind of silly but won’t kill my other households. It’s less that I care Rando McRanderson died—although you would think that would drag down my restaurant rating, if so please don’t—and more that I don’t want MY Sims to die because one of my other households went to a restaurant that didn’t even serve that item. Also I’m very offended that my four star restaurant is allegedly serving poor quality food. Also, did I mention that we don’t serve pufferfish at all?

(We did save Rando with a death flower, btw; this household had one.)

What expansions, game packs, and stuff packs do you have installed? All except goth kit, bistro kit, and pool lounge kit.
Steps: How can we find the bug ourselves? My sim (to my knowledge) NEVER ate pufferfish nigiri, but keeps dying from it at random after eating other cooked meals. She cooked it poorly once, but another sim in the household ate it. I believe she was pregnant at the time, so if she had been queued to eat it that might be the source of the bug—she never died and now it triggers randomly since she's no longer pregnant. She doesn't die after every meal, but garden salad, grilled cheese, and berry waffles among others (On multiple different lots) have triggered the "from pufferfish" poisoning death.
What happens when the bug occurs? The sim dies from bad pufferfish nigiri despite eating a different cooked meal. She eats, the death triggers, and I have to revive her in order to keep playing.
What do you expect to see? The sim does not die from eating regular cooked meals.
Have you installed any customization with the game, e.g. Custom Content or Mods? Not now. I've removed them.
Did this issue appear after a specific patch or change you made to your system? Neutral/Not Sure

The only mods I use are cosmetic (Loading screen color change) and CAS items like makeup, hair and clothes. There is no pufferfish in her inventory nor in the fridge. She has moved household lots since the error began happening.
