It does seem to be a timing thing. One of my children got sick and was showing signs of illness. She was able to take medicine *after* she got a sick buff.
This is different from sims who are teens or older. They can show signs of being sick but can take medicine *before* they get the sick buff.
I wish sim kids were the same. But at least my kids aren't stuck being sick, I just have to wait about a sim day for them to get a sick buff and then they can take medicine. Not sure if this is a bug or by design.

I’ve had this issue myself, why I just googled this and found this thread. Just for shites and gigs I took the medicine out of the child sim’s inventory and placed it on a surface and boom! The option to take medicine appeared.

I'm getting this too, my sim children can only take the medicine if the sick moodlet is the one that is currently dominate, so if they have the itchy buff, they have to be uncomfortable with it, if they are say focused with the itchy buff in the background then they can't take the medicine. Tried buying new medicine, trying to take it straight from inventory, put it down on a surface but they don't get the option to take it unless the buff is the most dominant one at the time. Hope that makes sense.
