I just got this bug tonight playing on XBox Series X. Since I can't share the save from console, here is what happened.
I had a family that consisted of:
Sim A (Adult)
/ \
Sim B (Young Adult, daughter of A) Sim C (Young Adult, son of A) -- Sim D (Young Adult, fiancé of Sim C)
/ \
Sims E, F, G (Children, sons of C) Sim H (Infant, son of C)
When Sim H aged up from infant to toddler, his random outfit was hideous. I went into CAS from Manage Households and edited the outfits of all the kids. 1-2 game days later, I noticed that my engaged Sims were showing up as both "Lovers" and "Brothers" in the relationship panel and all romance interactions except Offer Massage were not available.
I also noticed that despite Sim H being listed as Sim C's son, I still got the option to "Adopt as Caregiver" so I wonder if all the relationships have gotten messed up? I can provide some screenshots if needed.
This is a fresh save and I imported the family from my library. This save has never had any cheat codes used and of course no mods since I'm playing on console.
