This might be because accidental deaths are triggered depending on population size. They have a much higher chance to occur when the population reaches the max limit. And depending on how many packs someone owns and on play style this might take a little longer. I often start playing with a totally wiped save and it always takes a while until the first deaths by accident occur.

@flauschtrud So you're saying that I should try to hit three or more gens? Do you know how long it takes? Because I've gone two gens with none recently.
- flauschtrud2 years agoSeasoned Ace@Fleeglesnark hmm, I don't remember exactly how long it took me... How many Sims do you have in this save? Including random townies?
- Fleeglesnark2 years agoSeasoned RookieThe max size is set to 80, but I only ever play the one family. If I go into the other households, I always mark them as unplayed after I'm done. All the settings have been reset to default multiple times, just in case I missed a setting that I might have accidentally changed. But, as I said up there ^^, I've also played with only deaths on, and it jsut doesn't do anything. This has happened for months now, through all my saves since about the start of the year, if I remember correctly. On both Xbox One and PS4 now. I had one family on normal lifespan get to gen 4 with no accidental deaths. I have one now who is on short lifespan and is about three or four days after aging up to adult with no deaths. There's a new save from the other day on normal lifespan about two weeks in in-game and none there either.

Idk if this is accurate. I have 1092 sims in my save, NS stories isn’t taking them out anymore so the population is growing kinda outta control.
