@DragothX1X for the lower levels 1-3, your sim acts as the orderly role, so there will be a nurse and doctor there with you at the hospital.
For the mid levels 4-6, your sim acts as the nurse, so an orderly and doctor will be at the hospital for you to transfer cases to.
For the higher levels 7+, your sim acts as the doctor, so there will be a nurse and orderly there. There shouldn't be any tasks that involve transferring a case. At these levels, there are occasional days when a message pops up that you are short staffed, and that means an orderly hasn't shown up - on those days you might see an extra doctor at the hospital instead.

@simvasion I had a task yesterday that was transfer a case and I mean its kinda hard to do that when there is no other doctor working
