Got the same issue in my brand new game save. Discussing other fears such as 'stuck in dead-end job' fulfil, but discussing 'fear of death' does not, and the fear won't go away.

I did not notice the pattern but now it fulfils sometimes, maybe after doing the interaction 3 or 4 times, but I cannot say it works for all Sims and all the time. I had put the mods back now so I cannot send the save. But I have screenshots. Young adult Sim talking to an Adult Sim. The action is not fulfilling, but she gained a sentiment from talking to her.
The thought bubble you’re hovering over describes what you need to do to get rid of the fear - Click on a Ghost or Elderly Sim or obtain a Death Flower and give it to the Grim Reaper.
