If you have a sauna in your house/hangout spot + you have a club with a club activity set to "be friendly to," sims will woohoo in the sauna with each other. For some reason woohooing in the sauna is considered a "friendly interaction." I know this because when the only club activity is "be friendly" the option to woohoo in the sauna has a thumbs up symbol next to it. It's sort of hilarious but a bit problematic when it comes to certain groups, as you can imagine.

EDIT: Yap, it seems the Sauna that's the issue.
Still an issue as of the latest patch. Some sims have even cheated despite being happily married (One of my active families in rotational play, I might add 😠 ) while others have broadened their sexual orientation.
LOL, I'm glad to see other people are having the same problem I had. My club is for my legacy family, and they gather together often. I happened to notice a woohoo buff on the wife, but her husband had been at work for several hours. I checked her relationships and noticed she had romantic relations with both her mother-in-law and her daughter-in-law. The women were getting together in the sauna, because all beds/closets and other woohoo objects weren't accessible to non-household members.
It was an easy enough situation to fix, but it shocked and amused me when it happened.
