Steps: How can we find the bug ourselves? Have a sim tan and then their portrait will change.
What happens when the bug occurs? Their eyes get super big and their nose small
What do you expect to see? For their portrait to not change
Have you installed any customization with the game, e.g. Custom Content or Mods? Not now. I've removed them.
Did this issue appear after a specific patch or change you made to your system? Neutral/Not Sure
Example
I have the same problem, in my case my Sims after the tan the get a huge chin.
