I can confirm that I had the same thing happen. I had my child sim issue the positivity challenge and get access to the aspiration but she did not complete it before she aged up to a teen (she was on tier 3 or 4, I can't recall exactly which it was). The aspiration disappeared and when she called the event coordinator, her only option was to ask about the challenge, not to issue i
t again.

I add Teen to the household and it seems to be fine that this sim can issue
So i think the problem is aging up while the challenge didn't finish
